• Please print out the following forms and have each parent sign as part of the initial team meeting:

  • Parents Code of Conduct
  • Concussion Acknowledgement
  • Coaches Checklist
  • Residence Eligibility Requirements

• Any Parent that will be helping run drills or has contact with the players will need to register online and have a background check run.
